Faith Community Clinic provides affordable access to healthcare services and prescription needs for our uninsured neighbors. Faith Community Clinic offers, FaithCare, an affordable alternative to a conventional health insurance plan. Monthly plans start at $29/month and are based on income.

  • Proof of residency or employment in Stone or Taney Counties (Rental contract, utility bill, property tax bill).

  • Tax records (most recent 1040 or be prepared to complete the 4506-T Form).

  • Income records for household (two most recent paycheck stubs or P&L and proof of unemployment, disability, and child support).

Description:

For individuals who are pregnant and denied coverage through MO-Health Net, this program will step in to provide coverage for the unborn child and mother. 

 

Services Provides:

Pregnancy-related care for up to 60 days after birth,  health care services for the child till one year of age, 

and home visits, breastfeeding consultation, and a breast pump.

 

Other Important Information and Documents Needed to Register for Show-Me Healthy Babies:

  • Must be pregnant to apply for MO HealthNet (Missouri Medicaid) before delivery.

  • Live in Missouri (a Social Security Number is not required for non-citizens) 

  • Have a household income up to 300% of the Federal Poverty Level.

  • Not have access to employer or private insurance that covers pregnancy-related care. Not eligible for any other MO HealthNet programs.

Disabled American Veterans Transportation Services (DAV)

Description of Services Provided:

DAV operates a fleet of vehicles around the country to provide free transportation to VA medical facilities for injured and ill veterans. DAV stepped in to help veterans get the care they needed when the federal government terminated its program that helped many of them pay for transportation to and from medical facilities. Volunteers drive the vans, and the rides are coordinated by more than 161 Hospital Service Coordinators around the country.
